- Wellness Assessment Get $75 tax free ($150 if your covered spouse also participates) for filling out a Wellness Assessment between January 1, 2010 and March 31

- SHPs provides Health Management Programs including Maternity Care, Disease Management, Case Management and Precertification for hospitalization as well as many other services.

- Healthy Living's Employee Assitance Program (EAP) is a confidential service available 24/7 for help with issues such as anxiety, depression, substance abuse, marital, family,and relationship conflicts and more. Call 1-800-646-5608 to speak to a professional counsler. You and your family have unlimited access to phone counseling sessions.
